Your Care Plans

For the Family: The Initial Care Plan is Family-Tailored So YOU Can Make a Difference

Shortly after admission to our facility, the Interdisciplinary Care Team (IDCT) will meet with you and your family members to develop a Care Plan. Each discipline (Nursing, Therapy, Nutrition, Activities, Social Service) of the IDCT will be present at this meeting to make sure you receive the unique services your individual situation requires. A broad spectrum of information is taken into consideration:  medical data (diagnosis and prior level of function); personal information (likes and dislikes, religious preferences, social and family support); personal history and interests. In instances when a patient isn’t able to communicate effectively, family members are relied-upon for key information.

Getting Started: The Care Plan Meeting

Our objective is to have the patient and family participate in the development of the Care Plan to the fullest extent.

The Care Plan meeting is typically conducted within the first or second week after admission, and involves:

  • The Attending Physician
  • Registered Nurse/Nurse Representative/Director of Nursing
  • Dietary Services Supervisor/Registered Dietician
  • Director of Social Services/Social Worker/Social Services staff responsible for the resident
  • Activity Director/Activity staff responsible for the resident
  • Director of Rehabilitation/Rehabilitation Specialist/Physical Therapist/Occupational Therapist/Speech Language Pathologist, as needed
  • Nursing Assistant responsible for the resident

As a concerned family member, this meeting is an opportunity to get your questions answered. We understand what a worrisome process this can be, so we carefully explain the details of what to expect during your loved one’s stay with us, and how you can assist in their recovery.

Frequent updates on progress, improvements or adjustments in the Plan of Care are always given, to keep families well-informed and free from concern.

Get Well and Go Home: The Discharge Plan

Discharge planning is initiated once the Care Plan is developed by the IDCT.

The Discharge Plan provides a “road map” for bringing a resident home, functioning at the highest level possible. Included in the discharge planning is the resident’s Discharge Outcome Goal.

The Discharge Outcome Goal is based upon the professional opinions of the IDCT during their initial evaluation, and is always open for necessary changes indicated by a patient’s progress. It will be developed with resident and/or family objectives in mind:  destination of the resident, sources of assistance available, level of needed assistance, any adaptive equipment necessary, and appropriate required referrals.

Home or Away, We’re Still Here to Help: The Outpatient Care Plan

Outpatient programs are designed for patients who can return to our clinic for continued therapy. Our Outpatient Therapists are usually the same therapists the patient became familiar with during their stay with us, so follow-up is most-effective.

Additionally, our Therapy programs often involve local Assisted Living Centers and Senior Centers, providing wellness classes, ongoing training and education, and health screenings.

At Keystone Care, we believe that family involvement is crucial to the overall success of a loved one’s stay with us. Our team of medical professionals will work with you and your family to ensure the very best care for the duration.

We do this at the outset by implementing a thorough information-gathering process:

1The initial in-person meeting and interview, where we collect important information about you and your needs and preferences.

2You and your family, working with our interdisciplinary team of medical professionals, will design a Total Care Plan that works toward expediting your recovery.

3Formulation of the Discharge Plan, creating a “road map” for returning home successfully and functioning at the highest level possible.

4Implementing an Outpatient Care Plan for patients who are able to return home, but still need to visit the facility periodically for therapy treatments or other outpatient care.

Your Interdisciplinary Care Team

Many Disciplines — One Goal

The Interdisciplinary Care Team brings many together to achieve one goal: a patient’s recovery. This team is comprised of experts in the areas of focus within our facility: Nutrition Services, Social Services, Nursing and Rehabilitation. The team meets each week to discuss how patients’ needs are addressed, and to make sure they’re met. We take ownership of your care, and are accountable for the results.

Once the patient is on the road to recovery, it’s time for the Rehab Therapy team to take over. Consisting of Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), Certified Occupational Therapy Assistants (COTA), a Speech and Language Pathologist (SLP), and a Rehab Aide/Technician, this team works together attending to the individual needs of each patient.

The Physical Therapist assesses and treats impairments in strength and function that occur as a result of injury, disease, age-related degeneration, or environmental factors.

The Occupational Therapist assesses the impact that a patient’s decline in function has in their ability to perform everyday tasks such as dressing and bathing, returning to work, or engaging in previous hobbies.

The Speech and Language Pathologist assesses language, cognition, and swallowing dysfunction.

These three disciplines work as a team with Nursing, Social Services, Activities, the Business Office, families, caregivers, and local communities to serve a patient’s individual needs. Family members are essential contributors as well, providing the background and details of their loved one’s life.

All of our Rehab Therapists are full-time and in-house, and their relationships with residents are a mark of our outstanding care. We strive to build a strong trust with our residents; there is comfort in knowing that the same smiling face will be there to help them through their rehab each day.

We Don’t Wait To Make Important Decisions

Providing the best care possible dictates that Caregivers be empowered to make decisions—in many cases, every second counts. We leave the critical decisions to those who know best: the Caregivers, Doctors, and Nurses who work with our patients 24 hours a day, seven days a week.

It’s important to us that our patients, as well as their family members, feel loved, informed and secure during their stay. Quality of life is just as important as quality of care in a skilled nursing facility! We work hard to keep patients happy and comfortable, and patients have consistently shown greater motivation in achieving their goals as a result. If there’s a special need or request, the Team Member has authority to act upon it.
